Smell great and I don't mean wearing a butt load of cologne, even if it is a good scent on you. Clean cut looks, sensual full lips, someone who has good eye contact when speaking to me, attentiveness and affectionate tendencies.
For me, a first date no-no is:
Having sex, sleeping over, eloping or moving in with each other. :-D

If I won the lottery and quit my job, I would:
Buy a small home, take care of family, travel the world and start different charity organizations to aid in different causes I believe in.

I'm not sure since she is no longer with us. I would hope that she was proud of me.

A clean house that smells great!! I try to keep it that way but it just doesn't stay that way.
